After being operated on and suffering a stroke during the surgery, Lassiter wakes up in the Herschel House recovery home only to see Shawn and Gus standing over him. The duo brought Morrissey with them as a gift, who is no longer Karen's because he apparently kept biting Iris. Lassiter's nurse, Dolores O'Riordan (Sarah Chalke), comes in and asks to speak to Shawn and Gus privately. She reveals that she knows the duo have other motives in visiting Lassiter, and they admit that Lassiter has told them about mysterious goings-on at the home and wants them to investigate. Although she feels very attracted to Gus, Dolores says that it is only natural for someone in Lassiter's drugged-up state to be seeing things and orders the duo not to feed into his fantasies.
Shawn and Gus arrive back in San Francisco and start preparing a romantic dinner for Juliet. Gus hides when Juliet comes home to leave the two love birds alone, but only a short time later Woody calls him with the hand results. Shawn distracts Juliet and joins the call. The duo learn that the hand belongs to Devon Tileback (Antonio Cayonne), the CEO of a technology company who apparently committed suicide by jumping into the Santa Ynez river. Shawn tells Gus that they need to go back to Santa Barbara to solve the case before Juliet finds out about the hand and gets involved even further.
Lassiter wakes up in the middle of the night with Morrissey lying on top of him. He tells Morrissey that he is afraid he will become a burden to Marlowe and Lily, as the SBPD will probably retire him and he might never even be able to walk again. Suddenly the two hear a sound outside, and when they look out of the window they see a bleeding man stumbling for the home's equine therapy stables. Lassiter fears his nightmares are causing him to go insane, but his father appears beside him in a hallucination to reassure him.
Everyone is fast asleep, but Shawn suddenly wakes up to the sounds of a bumbling baby. He walks into the hallway and discovers Mary as a baby in a jumper chair. Mary expresses the joy of being a baby to Shawn, who sings him asleep with a lullaby. Gus has followed Shawn into the hallway and can also see Mary, who disappears when Wilkerson comes sneaking past and distracts the duo. Alarmed, the duo chase him down and confront him about his fake coma. Wilkerson admits that he has been able to function more or less normally for quite a while but decided to hide it for his own protection. He also says the patients at the home are somehow fed psychotropes, which causes them to hallucinate and confess their secrets more readily. Shawn remembers his and Gus's accidental confessions to Herschel in the stables and realizes the psychotropes are inside of the ice chips. He also realizes that Wilkerson and Tileback were in business together, and Gus understands that this must mean Wilkerson is the silent partner. Wilkerson reveals that he and Tileback had an IPO coming up and had to cover up their cooking of the books. He got panic attacks from the stress of hiding it and suffered a stroke. He was brought to the Herschel House, where he admitted the crime to Dr. Herschel under the influence of the psychotropes. Some days later, the silhouetted man that would go on to shoot Lassiter went to Tileback's office and blackmailed him. This worked until Tileback wanted to meet up with Lassiter. Lassiter was shot and Tileback fled but was eventually found and killed as well. Shawn and Gus now know that Dr. Herschel must be working together with an outside man who shakes down the home's patients using the information extracted by Herschel. 59ce067264
